Eligibility Criteria
- Must be a registered ClearMark Health member
- Must have a diagnosed chronic pathogen condition, post-infectious syndrome, or related chronic illness
- Requested therapy must be recommended by a licensed healthcare provider
- Must demonstrate financial need (insurance denial or lack of coverage)
- One active application per member at a time
- Grant amounts typically range from $500 to $5,000
